Our Person-Centered Care plan is coordinated by meeting with the resident and his or her family before they move in to design our care to match individual needs. The care that is given is flexible and is changed when the resident’s needs change.

We make a customized service package to meet the needs of each person, and new services can be added a la carte as needs change. The Oak Park Arms works with several excellent home health agencies and medical practices, all of which are Medicare certified. Physicians agree that most patients recover sooner and more comfortably in the familiar surroundings of their home. Sanders & Warren Geriatric Care Management provides independent professional care managers with expertise and services to ease the strain associated with caring for an elderly or disabled family member. The Oak Park Arms has a Wellness Center staffed by a registered nurse available to assist Arms’ residents with medical needs such as blood pressure monitoring, medication education, monitoring health conditions. Professionals from Interim Healthcare at the Oak Park Arms are available to residents who are dealing with acute illness, long term health conditions, permanent disabilities, terminal illness or the impact of aging. Medication assistance at the Oak Park Arms includes medication reminders to ensure medication is taken at proper times. Two dedicated internal medicine physicians spend time each week at the Oak Park Arms retirement community seeing residents in the wellness office leased by West Suburban Hospital. Both physicians, Kenneth Blair, M.D. and Brent Cutro, D.O. have extensive experience in the care of geriatric patients, and Dr. Blair has a subspecialty in geriatric care. A discrete, waterproof, personal emergency alert button is available to all residents of the Oak Park Arms. Once activated, the button sends a signal to the staff alerting them that a resident is in need of urgent care after a fall, sudden illness or any emergency situation.
