Geriatric Care Management
With more than 25 years experience, Sanders & Warren Geriatric Care Management provides independent professional care managers with services and the expertise to ease the strain associated with caring for an elderly or disabled family member. A care manager is a professional who evaluates, advises and guides seniors and their families through the maze of long term care services and provider options. Geriatric care managers are licensed professionals, primarily social workers, counselors and nurses. Care managers have experience, industry contacts, and knowledge of public and private services in the community, along with being thoughtful listeners and creative problem solvers. The care managers of Sanders & Warren start with a comprehensive, individualized evaluation. Based on results, services are recommended that will provide improved health, safety and happiness for your elderly family member. We also continually monitor all services provided, and, on an ongoing basis, give the family an updated personalized health care status report.
The Wellness Center
The Oak Park Arms has a Wellness Center staffed by a registered nurse available to assist Arms’ residents with medical needs such as blood pressure monitoring, medication education, monitoring health conditions to help residents decide if an emergency room visit is necessary or ordering medical equipment. The registered nurse answers a resident’s medical questions, and he or she addresses concerns. The nurse is also a liaison between residents and their physicians.
Professionals from Interim Healthcare at the Oak Park Arms are available to residents who are dealing with acute illness, long term health conditions, permanent disabilities, terminal illness or the impact of aging. Home health care personnel include nurses, therapists and other specialized professionals who provide clinical intervention such as wound care, assistance with medication, improved ability to walk, or teaching residents and their families’ self-management of their condition while remaining at home. Residents remain safe, independent and engaged in leading enriched lives. It’s often the kind of in-home help that allows people to avoid moving to a nursing home.
Since 1995, Podiatrist Maria Chon has had a private practice located at the Oak Park Arms. Her impeccable care and gentle nature make an exceptional combination, and the location of her office affords great convenience to the residents of the Arms. Her practice is open to the general public as well. Dr. Chon has special knowledge and expertise in diagnosing medical problems and management of diseases, disorders, deformities and trauma of the foot and ankle. She works with many older adult patients which is an important part of her mission. She earned her degree from the Dr. William Scholl College of Podiatric Medicine graduating Magna Cum Laude. Following the completion of her residency at Loyola Medical Center, she went into private practice at the Arms.
With more than 40 years of experience in audiology, hearing aid evaluations and hearing aid fittings, Dr. Robert C. Beiter, Ph.D., has been providing services at the Oak Park Arms since 2000. Dr. Beiter’s reputation for quality and responsiveness makes him invaluable to residents. His philosophy is to make good hearing an economic reality for all people. The Hearing Place serves the residents of the Oak Park Arms as well as people living in the Oak Park area.
